PLAYER/DIVISION ELIGIBILITY EXCEPTION REQUEST
The purpose of this form is to request an exception to the player/age divisions as set forth by US Lacrosse and adopted by the Rhode Island Youth Lacrosse League. Exceptions to the established age divisions will be considered in situations where it is in the player’s best interest to ‘play-down’ in a younger division due to physical, developmental or other mitigating circumstances.

US Lacrosse has established the age eligibility guidelines for the purpose of creating parity and competition with/between
comparable age and youth development parameters. All programs will have players whose DOB places them at the top end or bottom end of a given age division. Simply “just missingthe cut-off” is not cause for a player exception - other circumstances must be present.

Player exceptions will be limited to Division II (recreational) players only. No exceptions will be considered for Division I (competitive) division players. This form is to be completed by the requesting program’s President or Program Director ONLY.

OUT OF ZIP CODE ELIGIBILITY EXCEPTION REQUEST
The purpose of this form is to request an exception to the player's zip code boundaries as set forth by Rhode Island Youth
Lacrosse League. Player exception for playing outside the boundaries drawn up by RIYLL are limited to the rules established
by RIYLL under the heading 'Playing Out of Zip Code'. This request is not an approval and will be reviewed by the
Competition Committee.

All waiver approvals are for 1 year only and must be request on a yearly basis. The waiver is to be completed by the requesting program’s President or Program Director ONLY.
